Chosen Path Home Care provides Non-Medical Home Care. We assist seniors and people with disabilities or illness with their activities of daily living such as bathing, grooming, dressing, meal preparation, light housekeeping, medication reminders, transportation, ambulation etc. Home Health Care Agencies provide medical/clinical care such as skilled nursing, therapy, medication administration, wound care, etc. Compassion Home Care is NOT a Home Health Care provider.

Yes. We can provide you with your own personal, private caregiver in assisted living and nursing home facilities.
Yes, we are a licensed provider and bonded as required by the state of Texas.
Yes. To help ensure the safety and well-being of our clients, we perform thorough background checks on each employee. We also conduct comprehensive personal & professional reference checks for all agency employees.
Will i have a choice of which caregiver comes to my home?
Yes. Based on your personalized Care Assessment, we will match available caregivers with you, and you will have the opportunity to meet them prior to assignment.
This depends on your caregiver’s schedule. We understand that most people prefer to see the same friendly, familiar face regularly. However, caregivers also need personal time. We will do our best to respect and maintain your existing caregiver relationships and accommodate your requests. In certain circumstances, a relief caregiver may be assigned, but rest assured, we’ll do everything we can to ensure you, and your family remain comfortable and happy.
Yes, you may change, cancel, reschedule, or discontinue any home care services, provided you contact Chosen Path Home Care management at least 24 hours in advance and make arrangements with one of our Care Managers. If we do not receive notice of a rescheduling, cancellation, or discontinuation less than 24 hours before a scheduled shift, fees may apply.
We understand that care needs can change quickly, so we strive to be as flexible as possible. Please reach out to us promptly regarding any last-minute schedule changes, and we’ll do our best to accommodate them.
No, you don’t need pre-authorization or a prescription for Chosen Path Home Care Services. You have the freedom to choose the services you need, when to start them, and how often they are provided.
Depending on your private insurance, it’s possible that your health insurer may cover some or all of the services you receive. It is your responsibility to contact your insurance company to determine if they will cover our services. In this case, you would pay Chosen Path Home Care directly when invoiced, and your insurance provider would reimburse you for the payment.
Home Care costs depend on the specific needs of each individual Client. The cost of services are addressed with each and every Client after we assess your needs and develop your custom Plan of Care. Several factors to consider are whether your caregiver is on an hourly schedule, full time live-in schedule or you simply need overnight care. An agreed upon written rate agreement will be provided to you prior to any caregiver services being rendered.
No. You (or your authorized representative) should never pay your caregiver directly for any of your Home Care services. All payments must be sent to Chosen Path Home Care once you are invoiced. If your caregiver ever requests direct payment from you please call management immediately
We are required by law to maintain the privacy of all information provided in our Client records. Please refer to our “Notice of Privacy Information Practices” contained within our Client Service Agreement for further information as it pertains to how we may use and disclose protected health information about you. Protected health information means any health information about you that identifies you or for which there is a reasonable basis to believe the information can be used to identify you. Your information is never sold to any third party under any circumstances and Chosen Path Home Care is in full compliance with all HIPAA guidelines and regulations.

Together, we’ll go over the care your family needs and the care we provide. We’ll answer all of your questions regarding in-home care services and costs. Then, we’ll begin putting together a personalized Care Plan and matching your loved one with the right caregiver. Usually, care can begin within a matter of a few days.
